Beyond Blame: Understanding the 12 Hidden Triggers of Human Error

What happens when we assign “Human Error” as a root cause? Can the corrective action be effective?

Rather than assigning fault, this presentation focuses on understanding why mistakes happen. Using the “Dirty Dozen” framework, it reveals twelve hidden triggers — such as complacency, pressure, lack of awareness, and inadequate communication — that commonly lead to errors. Attendees will discover how a culture of openness, feedback, and psychological safety can transform how organizations respond to human error. The goal is to shift from reactive blame to proactive prevention, building a workplace that learns and improves continuously.

Chris Cinieri has more than 20 years of manufacturing experience working in quality and operations. Within his role as a manufacturing consultant, he helps companies create, leverage and audit Quality Management Systems, delivers effective leadership training and facilitates strategic planning. Chris has a Bachelor of Science degree from Rhode Island College and is a certified Six Sigma Black Belt from the American Society for Quality where he serves on the local ASQ leadership team.
